As a hobby I have collected over the years many Apple II items (mostly from eBay in its early years) and they are taking up space. To make room for my family I have sold some items here a while ago and I am now selling more of them. My first computer was an Apple II+ clone and so I know something about these items. I'll try to answer questions to the best of my knowledge.
This is the No-Slot Clock from Dallas Semiconductor circa 1984. As its name implies, it does not required a slot as other clock cards do and is installed between a 28-pin ROM chip and its socket in an Apple IIe or IIc. More information about this clock chip is available here. I purchased this brand new (from Syndicomm) and have not used it so the battery inside is fresh (The embedded lithium battery was electrically disconnected until power was first applied to retain freshness). A historic item great for the Apple II collectors and enthusiasts.
